DNS ( Deviated Nasal Septum ) | CHAPTER-10 | Medical and Surgical Nursing

DNS ( Deviated Nasal Septum ) – This course is designed to understand the concept of community health nursing: nurses’ roles and interventions in family health, school health, occupational health, environmental health, elderly health care, gender issues, disaster management and principles and terminology of epidemiology. The aim of the course is to acquire knowledge and skills in community health nursing.

DNS ( Deviated Nasal Septum )

DNS means shifting of the septum from midline in one or in both sides, partially or completely, producing asymmetry in nasal cavities with or without external deformity.

Or,

A deviated septum occurs when the thin wall (nasal septum) between nasal passages is displaced to one side. In many people, the nasal septum is off-center-or deviated-making one nasal passage smaller.

Classification of Deviation Of Nasal Septum:

➤ Anterior septal dislocation

➤ ‘C’ shaped deformity

➤ ‘S’ shaped deformity

➤ Septal spurs

➤ Septal thickening

Types of Deviated Septum

Causes of DNS:

➤ Congenital:

  • Birth molding theory – abnormal position.
  • Variation of rate of growth the two sides of the facial maxillary region.
  • High arched palate and cleft palate.

➤ Trauma:

  • Birth trauma (Primi para, prolonged labour, forceps delivery).
  • Boxer and players.
  • Fracture nasal bone by various accidents:

✔ RTA (road traffic accident)

✔ Fall from tree etc.

➤ Genetic/racial.

➤ Idiopathic.

Management of DNS:

Clinical feature:

➤ Symptoms :

  • In majority cases minor DNS having no symptoms.
  • Nasal obstruction

✔ Unilateralienet

✔ Bilateral.

✔ Partial.

✔ Complete.

  • Pressure headache: Due to compression of turbinate (Ethmodial nerve).
  • Nasal bleeding or epistaxis.

➤ Signs:

  • Deviation of the nasal septum:

✔ ‘C’ shaped.

✔ ‘S’ shaped.

✔ Anterior septal dislocation.

✔ Septal spur.

✔ Thickened septum.

  • Congested mucosa: Only mucosa of deviated part of nasal septum.
  • Hypertrophy of inferior turbinate (HIT) opposite side & roomy nasal cavity.
  • External deformity.

➤ Investigation: X-ray PNS occipito-mental view

Deviated Nasal Septum

Treatment:

➤ If asymptomatic: Does not require any treatment.

➤ If symptomatic:

  • Below 16 years of age: Septoplasty.
  • Above 16 years of age: Submucous resection (SMR)
